Dr. Mundo runs on health instead of mana, and that single choice shapes everything about how he's played. Infected Cleaver costs a chunk of his own health to throw but deals damage based on the target's current health, so trading health for health against someone with more of it to lose is usually a good deal. Adrenaline Rush keeps him topped off passively between fights, and Masochism actually gets stronger the lower his own health drops, which flips the usual survival math on its head. Burning Agony adds a steady area toggle that also cuts down how long crowd control sticks to him. Sadism is the payoff for playing recklessly all game: dump health for a burst of speed and heavy regeneration right when it matters. He's slow to get going and vulnerable to poke that outpaces his sustain, but once he's rolling he's exhausting to actually finish off.
Counters & matchups
Sadism outlasts sustained damage; Madred's Bloodrazor and Vayne's true damage are built specifically to beat his health bar.
